What you need from your GP
- A valid referral from your GP on a D904 request/referral form — this is required before treatment can begin.
- The referral covers up to 12 sessions or 12 months, whichever comes first. There's no limit on the number of referral cycles you can have.
- Bring your Veteran Card to your first appointment.
Gold Card vs White Card
- Veteran Gold Card — covers clinically necessary chiropractic care for all conditions.
- Veteran White Card — covers chiropractic care for your accepted service-related conditions.
Good to know
There's no gap or out-of-pocket cost for eligible, clinically necessary treatment. If you've seen another chiropractor, physiotherapist or osteopath in the past 12 months, let us know — you can't receive these manual therapies for the same condition at the same time. When your referral cycle ends, your GP can simply issue a new one if ongoing care is needed.
